Welcome Kindergarteners to Mary Walter Elementary School!
In order for your child to be enrolled...
- He or she must have a comprehensive physical by a licensed physician, nurse practictioner or physician assistant prior to the first day of school.
- We will also need the most current vaccination record. Please bring/fax this to the clinic for review. We must have this in hand before the child can come to school.
If your child has a chronic illness that may need to be addressed while at school, please let us know so we can get a Care Plan together. The care plan must be completed and signed by you child's health care provider. CarePlan forms for asthma, severe allergies, seizures, and diabetes can be found under Frequently Needed Health Forms on the school website site or can be picked up at the school.
Regarding medication administration, a parent/guardian must bring the medication to the clinic and complete a "Medication Authorization". Any prescription must have a doctor's order to accompany it. The form is available under Frequently Needed Health Forms on the web site, ore you may pick it up at school. Students may NOT bring in any form of medication including cough drops. You will take any remaining products/medicines home the last week of school.
Illness. Please make sure the information you put on your emergency card is correct and kept current. It is very important to quickly locate a parent to pick up their sick child. Please call the clinic or office number in the morning if your child will be out sick, you may leave a message on the clinic line (540) 422-7715.
